Initial exprience with the radiolabeked synthetic amino acid, antiF-FACBC, which displays slow urinary excretion has been encouraging but further studies will be needed to decipher its exact role in the imaging management of men with prostate cancer.
Another study showed that GLUT1 gene expression was significantly higher in prostate cancer than in benign prostatic hyperplasia. Although cases with scatter from the adjacent high bladder activity or residual urethral urine activity were excluded, but the study was somewhat limited with respect to the definition of normal prostate gland that was based on no clinical evidence of prostate pathology.
It is however well recognized that normal prostate tissue, benign prostatic hyperplasia, and small deposits of prostate cancer can all coexist in asymptomatic men. Experience has suggested that FDG PET may have generally a limited role in primary diagnosis and staging of prostate cancer in view of overlap of tracer accumulation in normal and abnormal prostate tissues.
The cut-off SUVmax, sensitivity and specificity for differentiating between biopsy specimens summed Gleason score of 5 or less and those specimens with summed Gleason score of 6 or greater were 2. A large number of these men present with negative standard imaging studies.
Appropriate treatment relies on early and accurate detection and localization of disease in these men. Salvage radiation or surgery may be considered for local recurrence while systemic therapy will be needed for metastatic disease. The overall experience with FDG PET in this clinical setting is relatively limited both in quantity and quality of studies.
However, other positive standard imaging already indicative of metastatic disease was used as validation for FDG PET in a number of cases when by definition in PSA relapse-only clinical state, the standard radiographic scans typically Tc99m-based bone scintigraphy and CT should be negative.
FDG accumulation at metastatic sites tends to decrease with successful chemohormonal therapy.
